Admission Process
- Doctor assesses the patient either in the OPD or casualty and advises admission when required. This admission could be on emergency / immediate basis for critically ill patients or on elective basis (on a convenient day) in non-emergency situations.
- Wherever possible the patient is counselled by the doctor regarding the treatment package which includes documents to bring on day of admission, average length of stay, estimated cost, various modes of payment etc. However the estimates may change depending on the complexity of the disease and due to unforeseen circumstances.
- Doctor writes appropriate admission note for the registration /admission staff.
- The patient / relative reports to the registration / admission counter on the stipulated day for paying the necessary deposit and making the indoor admission file after which the patient is directed to the appropriate ward.
- For critically ill patients treatment is initiated immediately while the relatives are directed to complete the admission formalities.
- Presence of one attendant is required with the patient throughout the stay in the hospital. One attendant pass is issued for the family member attending to the patient which should be carried by the attendant and produced on demand.
- Documents required for admission are as follows
- ID proof:Aadhar Card / Voter Card / Passport /driving licence.

- Additional documents may be required for patients admitted under ‘RGJAY’, ‘BPL’ and ‘Weaker Section’ schemes.
Discharge Process
- The treating doctor writes a ‘discharge note’ on the case paper.
- The patient/relative reports to the registration desk with the discharge note and is required to clear all payment dues and return the visitor and attendant passes.
- On receiving clearance from registration desk further discharge procedure can be completed by the patient / relatives with the help of the sister in-charge of the ward in which the patient is admitted.
- A discharge summary sheet and all investigation reports are handed over to the patient.
- A typical discharge process may take between two to four hours.
